Medical Information

Doane University Student Health Services is committed to providing holistic care to all registered, undergraduate students. The goal of Student Health Services is to keep students well so they are able to achieve their academic and personal goals. In an effort to do just that, newly accepted students at Doane are required to complete a health related history form and an immunization history through the Doane student health portal. All required information can be found under the “my forms” tab of your student health portal at Students will need to use their Doane email and password to enter the student health portal.

*Consent to treat- The consent to treat form is required to be signed by a parent or guardian AND the student if the student is under age 19. Please make sure both signatures are present.

* Physical Examination- A physical by the student’s physician is highly recommended for all new students and is required if the student will be participating in an intercollegiate athletic program. Sports physicals should be completed after June 1st.

* Immunizations- All students enrolled at Doane University are REQUIRED to be adequately immunized. This requirement is to ensure the safety of our entire campus from potentially dangerous communicable diseases.
